NEW U.S. PASSPORT

WHAT YOU NEED

1		Certified birth certificate or naturalization certificate or passport issued over 15 years ago.
2		2 Passport Size color photos (2x2 with white background). Click here for details .
3		Completed DS-11 Form. This form must be completed online. The passport office will not accept hand-written DS-11 applications. If the applicant has lost a valid passport, please complete DS-64 Form. Please include 2 copies of your completed DS11 form on the outside of your sealed envelope.
4		Government fees are $160.00 for adults and $145.00 for minors under 16 years old. These fees are divided into - Adult - $135.00 for “U.S Department of State” and $25.00 for the Post Office or the Clerk of Court. - Minors -$120.00 for “U.S Department of State” and $25.00 for the Post Office or the Clerk of Court. Travisa's Service Fees are - 3 to 10 working days processing - $99.00 - 24 and 48 hours processing - $179.00 - Same day processing - $199.00
5		Two letters of authorization. Please select the authorization letter that matches the processing location.
6		Proof of departure date- computer generated flight itinerary from the airline or a travel agency, copy of airline ticket or a corporate letter stating US departure by private aircraft.
7		Complete the Global Reservation Form. This is Travisa's Service Order Form. You must include a copy of this form with all other supporting documents. You must print this form and paper clipped it to the outside of the sealed envelope (see # 1 below).
8		Passports are processed according to U.S. Departure Date. You must call to reserve a space and fax a copy of the global reservation form to confirm your reservation (same day/next day/48 hours and 3 to 10 days processing). The service fee is non refundable.

WHAT YOU DO

Take the items 1-6 to Clerk of Court or Authorized Post Office and ask them to endorse the

documents and seal them in an envelope marked "TO BE OPENED BY PASSPORT OFFICE

PERSONNEL ONLY".

This sealed envelope must be presented to the passport office within 5 working days from

execution.

Do not write checks until directed to do so by clerk. ALL GOVERNMENT FEES MUST BE INSIDE THE SEALED ENVELOPE.

You must present proof of identity such as a driver’s license (which has been issued for at

least 6 months).

MINORS-All minors must appear in person at the post office or the courthouse.

Minors (under the age of 14) must be accompanied by BOTH PARENTS or legal guardian

with

acceptable proof of ID. If only one parent appears the other parent must sign and

notarize a

Statement of Consent Form DS-3053.

Do not leave documents with clerk.

You must submit the second completed application DS-11 for inclusion in the FedEx envelope, paper-clipped to the outside of the sealed envelope.

FedEx the Global Reservation Form plus the sealed envelope to TRAVISA for processing.

You must call to reserve a space and fax a copy of the global reservation form to confirm your reservation (same day/next day/48 hours and 3 to 10 days processing).
The service fee is non refundable.

3	Review the screen for accuracy.
4	Print the completed form by clicking on the print button from your web browser.
5	Review the printed copy to ensure the correct information.
6	Return it with all of the above requirements to TRAVISA.
