2 Visa Application Form: Two visa application forms per applicant properly completed and signed. Signature must be original on each application. Improperly completed forms may delay the processing of your visa.
3 Photo Requirements: One recent 2 x 2 passport type photograph(s), in color, front view and with a plain/light background.
4 Proof of Departure: Computer generated flight itinerary from the airline or travel agency, or copy of airline ticket or letter from travel agency.
5 Special Instructions: Applicants requesting Same Day service through the San Francisco office must provide an itinerary showing departure to China within 3 days.
6 Complete the Global Reservation Form: This is Travisa's Service Order Form. You must include a copy of this form with all other supporting documents. (NOTE: Consular fees are NON-refundable. If visa is rejected and resubmitted new consular fees will apply).
7 Fees

Consular Fee

Service Fee: - 24-Hours and same day processing are available for additional fees. Service fee for regular processing is $50.00. Rush service fee of $99.00 will be assessed for any attempts to process the visa in less than regular processing time.

Shipping Fees: The standard shipping method of completed documents is FedEx Overnight Service for which Travisa charges a fee of $21.95. Other shipping options are available when completing our Global Reservation Form.
  Visa Validity: Single entry visas are to be used within 3 months of issue. Double entry visas can be issued for 3 or 6 months, but usually 3 months. Multiple entry visas are based on the invitation for either 6 or 12 months. Time allowed to stay in China is 30 days for most visas.
  Visa Processing Time- Visas are usually processed in 5 working days. Rush and Emergency service may be available for additional fees. Please visit our Consular Fees page for details. Rush and Emergency service is defined as any attempt made by Travisa to retrieve the visa in less time than the amount required by the embassy to process the visa.

Non-refundable tickets or reservations should not be purchased until all visas and passports are secured and in your possession.

4 Official Invitation: Please note the following specifications for each jurisdiction:

Washington, DC: A letter of invitation from the host company/inviting organization –OR- a business cover letter from your US employer/sponsoring company. Chicago: A letter of invitation from the host company/inviting organization.
Los Angeles: No letter of invitation from the host company/inviting organization is required.
San Francisco: Official invitation (Visa Notification) issued by the Chinese Provincial Government. The invitation must have the Chinese Municipal seal on it. The visa notification must be original. Copies are not accepted.
Houston: A letter of invitation from the host company/inviting organization.
New York: A letter of invitation from the host company/inviting organization stamped with a government municipal seal. 
5 Proof of Departure: Computer generated flight itinerary from the airline or a travel agency, copy of airline ticket or a corporate letter stating travel plans. A signed letter on the travel agency’s letterhead confirming round-trip booking may also be submitted.
6 Business Letter: Letter from US based employer or sponsoring company, on company letterhead, introducing the applicant, indicating the applicant’s employment status/position held in the company, and clearly stating the purpose of visit to China. The business letter must also indicate who will be financially responsible for the applicant, provide detailed contact information in host country, and specify the number of entries being requested. If a multiple entry visa is requested, this must be indicated in the business letter and tentative date of future travel must be provided.
7 Special Instructions:  Applicants requesting Same Day service through the San Francisco office must provide an itinerary showing departure to China within 3 days.

6 Hotel Confirmation: A copy of confirmed hotel and/or cruise reservation(s.) If staying in various hotels, please provide copies of each reservation. A letter from inviting family member or friend is required if they are providing accommodations to the applicant.
Applicants within Chicago and Houston jurisdiction: If a family member is providing accommodations to the applicant, a letter and Certificate of Kinship (showing relationship) from the inviting family member is required. A copy of the inviter’s passport and valid visa/residence permit (if non-Chinese citizen) must be provided.
7 Other: Applicants within Los Angeles jurisdiction: Original Crew ID must be submitted.
8 Other Requirements: Applicants requesting a multiple entry visa to China must provide a letter stating the need for a multiple entry visa and the dates of future travel to the country.
9 Special Instructions:  Applicants requesting Same Day service through the San Francisco office must provide an itinerary showing departure to China within 3 days.

The holder of a (Z) visa shall go through residential formalities in the local public security department within thirty days of entry into China.
