6 Minors: A child (under 18) who is not traveling with both parents or a legal guardian must provide a notarized Letter of Consent signed by the non-accompanying parent or guardian authorizing the Brazilian consulate to issue a visa to the minor. Proof of guardianship must be submitted. Both letter of consent and guardianship document must be authenticated by the Consular Service of the Brazilian Consulate.
Please note: In addition Houston jurisdiction requires: - 2 application forms, each signed by both parents and notarized. - Notarized copy of minor's birth certificate. - Letter of Consent even if child is traveling with both parents. - For children 3 months to 6 years old, a certificate of vaccination against polio.

8 Financial Requirements:
Chicago Jurisdiction: Evidence of sufficient funds to support stay in destination such as pay stubs, traveler’s cheques, and/or most recent bank statement. Atlanta and Washington DC jurisdictions: Consulates in Washington DC and Atlanta jurisdictions may in some circumstances request evidence of sufficient funds to support stay in destination such as pay stubs, traveler’s cheques, and/or most recent bank statement. In Washington DC this is required for applicants who are entering and departing Brazil by land.
9 Special Instructions: Applicants within Chicago Jurisdiction: A host letter is required for individuals traveling to Brazil as tourists but not staying in a hotel. The letter must be signed and stamped with seal by a Cartório (Notary) in Brazil.
It should be from the host(s) or contact(s) in Brazil, addressed to the Brazil Consulate in Chicago and it should contain the following background information about the host(s) or contact(s) in Brazil: (1) Name(s), (2)Complete Address, (3)ID number if Brazilian citizen(s) or visa status and nationality if not a citizen, (4)Reason why the person is living or staying in Brazil (if not a Brazil citizen, (5)Relationship with the applicant (explain how you got to know each other), (6) Profession or occupation, and (7) Employer's name. In addition, the letter should provide an explanation of the purpose of your trip.

It is okay for this letter to be a copy (a fax for example), as long as the notary’s seals and / or stamps are legible. It can be in English or Portuguese.
10 Complete the Global Reservation Form: This is Travisa's Service Order Form. You must include a copy of this form with all other supporting documents. (NOTE: Consular fees are NON-refundable. If visa is rejected and resubmitted new consular fees will apply).

Service Fee: - 24-Hours and same day processing are available for additional fees. Service fee for regular processing is $50.00. Rush service fee of $99.00 will be assessed for any attempts to process the visa in less than regular processing time.

Shipping Fees: The standard shipping method of completed documents is FedEx Overnight Service for which Travisa charges a fee of $22.95. Other shipping options are available when completing our Global Reservation Form.
  Visa Validity: Tourist visas are usually valid for up to 5 years and allow a maximum stay of 90 days per entry. First entry into Brazil must take place within 90 days from date of visa issuance.

7 Business Letter: Original letter from employer or sponsoring company, on company letterhead, introducing the applicant, indicating the applicant’s employment status/position held in the company, and clearly stating the purpose of visit to Brazil. The business letter must also indicate who will be financially responsible for the applicant and must provide detailed contact information in host country. In addition, it is extremely important that the business letter indicates that "no technical work or assistance" will be provided during stay in Brazil. Note to applicants within Miami and Atlanta jurisdictions: Business letter must be notarized.

Document Legalization: Applicant's Birth Certificate and Good Conduct Record must be authenticated (legalized) by the Brazilian Embassy prior to the processing of the visa. The total fees for the authentication of these documents are: Brazil Consulate Authentication Fee - $30.00; Travisa Service Fee for Document Authentication - $54.00 ($32.00 for first document and $22.00 for any additional documents.) These fees are in addition to the Consular Fees and Travisa Service Fees for the processing of the Student visa. Please include these fees in your total payment to Travisa. 5 Official Invitation: Photocopy of the school registration or confirmation of acceptance, notarized parents' authorization (if applicant is under 18 years old), address/names/qualifications of responsible parties in Brazil.
6 Criminal Record: Good conduct record issued by the local (i.e., applicant's place of residence) police department/authority. Applicants within Los Angeles jurisdiction must provide a notarized copy of this document.

Temp II visas are usually valid for up to 5 years (in San Francisco visas are issued for up to 2 years) and allow a maximum stay of 90 days per entry. Notarized business cover letter addressed to the Brazilian Consulate explaining the purpose of the trip in detail, and the nature of duties to be performed in exact detail, declaring that "technical support" will not be performed in Brazil, and guaranteeing financial support for the applicant while in Brazil, and return transportation. If business in Brazil is NOT of a technical nature, you must specifically state this on the business letter.
